Meningococcal Vaccine
Attention parents of children entering seventh and twelfth grade in the 2021-22 school year - students must have the Meningococcal vaccine to enter school in September 2021.
Important information about physical examinations for the 2020-21 school year:
New York State Education Department regulations mandate that all students have a physical examination entering kindergarten, grades 1, 3, 5, 7, 9 and 11.
Each such certificate shall describe the condition of the student when the examination was made, which shall not be more than twelve months prior to the commencement of the school year in which the examination is required, and shall state whether such student is in a fit condition of health to permit his or her attendance at the public schools. The certificate should also state the student’s body mass index (BMI) and weight status category.
Physical forms and BMI information are available on the district website under parent information. Parents should submit their health care provider’s results to the school nurse on or before September 30, 2021. After this date, the physical will be conducted in the school by the district’s medical professionals.

We
want all of our students to have a healthy school
experience. The health office staff provides students
with the health services and testing required by the state education
department.
Our goal is to assist all students with their health issues
that they may enjoy and get the most from their educational experience.
The “Home School Connection” is especially important to us in the health
office. Communication about any health issue is encouraged and all
medical conditions and medication taken routinely at home should be
reported to the health office. Here are some other important things you
should know:
Required Immunizations
New York State law requires the following immunizations for school entry and attendance. As per public health law Section 2164, prohibits a school from permitting any child to attend school in excess of 14 days without sufficient evidence that the child has received all age-appropriate require vaccinations.

The number of doses depends on the schedule recommended by the Advisory Committee on immunization practices (ACIP). Please see the current school year requirements. If your child requires a medical excemption, it must be submitted on the New York State Medical Exemption form (DOH-5077), signed and stamped by a NYS physician and submitted for district approval by the medical director.
Prevention
Good health practices will improve your child's physical health, their behavior, school attendance and school
performance.
- Hand Washing (and general hygiene)
- Good Nutrition (especially breakfast. try to avoid snacks high in sugar.)
- 8-10 hours of sleep
- Appropriate clothing for the weather.

Physical Activity
Notification of Illness
- Please notify the attendance office if your child will be absent for an illness or injury.
- Notify the health office for flu or other contagious diseases.
- A note will still be required to excuse the absence.
Physical Education Excuses
Students may be excused from PE for illness or injury with a parent
note. Parent notes are accepted for the week in which they were written
for Elementary Students (grades k-4) and one day for the middle level
and secondary student (grades 5-12). Longer durations require an MD
note. Any child excused from gym will also be excused from outdoor
recess activities and sports. Medical clearance will be required to
resume Physical Education and sports.
