Sports Registration - FinalForms

The district now offers the convenience of online registration through Final Forms a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more administratively efficient and environmentally responsible. When you register through Final Forms, the system keeps track of your information in your Final Forms profile. You enter your information only once for each family member for multiple uses and multiple programs.

Private Care Physician:
Families choosing to obtain a physical examination by their private care physician may upload a copy of the completed physical examination form when registering on Final Forms for the Fall athletic season: https://longwoodcentral-ny.finalforms.com/   **For families new to Final Forms, included in this correspondence are directions on how to create a new account and begin registration. Physical Examinations: please utilize the most up to date New York State Physical Examination Form, which can be found at: https://www.p12.nysed.gov/sss/documents/health-exam-form.pdf.

School Appointed Physician:
 Families choosing to obtain a physical examination from the Longwood Central School District physician may receive an examination on the dates below.

BEFORE YOU REGISTER:

Please have your physical on hand so that you can upload a copy of it to your registration. You physical is good from one year through the month it was done.  For example if you received a physical on June 1, 2024 your physical will expire on July 1, 2025.

INFORMATION NEEDED TO REGISTER:

It will be helpful to have the following information handy to allow for the accurate completion of your online registration, including doctor information, health insurance, and student ID.

How do I sign up?

1.  Go to:  https://longwoodcentral-ny.finalforms.com/

2.  Locate the parent icon and click NEW ACCOUNT.

3.  Type your NAME, DATE OF BIRTH and EMAIL.  Next, click REGISTER

Note:  You will receive an email within 2 minutes prompting you to confirm and complete your registration.  If you do not receive an email, then check your spam folder.  If you still can not locate the FinalForms email, then email support@finalforms.com informing our team of the issue.

4.  Check your email for an ACCOUNT CONFIRMATION EMAIL from the FinalForms Mailman.  Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.

5.  Create your new FinalForms password.  Next, click CONFIRM ACCOUNT.

6.  Click REGISTER STUDENT  for your first child.
