Longwood Athletics Sport Clearance Procedures

New York State law requires all students participating in athletics to receive a sports physical. Sports physical information and forms can be picked up at the health office or by clicking on the 2 links below. The forms must be completed before your child is able to participate in the interscholastic program.  The results of the Sports Physicals shall be valid for a period of twelve months during the last day of the month in which the physical was conducted. The final approval for sports clearance rests with the school medical officer.

The Health Certificate/Appraisal Form is to be completed by your primary health care provider or by the school medical officer.  Make sure that your health care provider signs, stamps and completes the form including any medications, inhalers, etc. that your health care provider doctors order. A student may not participate without the approval of the school medical officer.

The Health History Form is part of the physical and is to be completed by parent/guardian and handed to the Health Office along with the Health Certificate/Appraisal Form. A new Health History Form must be completed thirty (30) days prior to the start of any additional sport in which the student would like to participate.

Interval Health History for Sports Participation Form must be on file and updated each sport season.
