Questions and Answers - The School Based Health Center at Longwood High School



1. What are the hours of the clinic?  Monday through Friday, 8:30am to 1pm.

2. Who can use the clinic?  What can the clinic help with?

Only Longwood HS students may use the clinic.  Acute or chronic conditions are reasons students are seen at the clinic.  Immunizations can be provided on request.  Prescriptions are called in, routine lab work (i.e., strep culture, urine) done, and referrals are made as needed.  That is all part of the service that would be provided without charge to the parent.  

3. How can a student be seen at the school-based Health Clinic in Longwood High School?
If a student visits the school nurse and it’s determined that he/she should be seen by a nurse practitioner, doctor, or PA, signed consent from parent will enable the student to be seen at the clinic. If not on file, a parent would be called and arrangements to receive consent by fax or email will be made.  Once a parent signs consent, a student can ask to be seen at the clinic.

4. Is there any charge to use the service?  
There is no charge to students or their families for medical care provided at the center. 
Insurance companies may be billed for visits, but no co-pays or other fees are collected from students/parents.   Staff will help families of uninsured children apply for coverage through New York’s Child Health Plus, Medicaid or other sources. 

5. What if a student needs medication?
 
Prescriptions are electronically sent to the pharmacy of the parent’s choosing.  In some cases, over the counter medication will be dispensed right there at the clinic. 

6. What happens after a student is seen at the clinic?

Parents receive a diagnosis in writing and prescriptions sent directly to the parent’s designated pharmacy.

7. How can a parent give consent for use of the clinic?
Complete the consent form with insurance information and provide a copy of both sides of the insurance/medicaid card (if student is insured).  Students who are 18 or older may provide their own consent.  

A medical history form and privacy permission form are also necessary.  All information and forms are located on the Longwood website.
